Hood has been used, It has some light scratches from use. But it is not damaged in any way, no cracks. This lens hood is in very good condition.   
 
Hood comes just as pictured. All pictures are of an actual item for sale. Hood ID is 59mm, It will fit lens with 55mm rim, it is too small for 58mm. I don't know what camera or lens it was made for. 

I would rate it an 9, on a scale from 1 to 10.
