This is Chikee Fan Blower Motor A33C351R 3400RPM MT21302 115/230V 60/50 Hz used for Teledyne Laars Jandy LXi 250 300 400 Pool Heaters motor.
This part was removed by technician/installer from a working unit. Motor has been inspected and tested, runs well.
