New old stock.  Fits phones approx. 5" X 2 1/2".  One open 1/2" slot on the left side and 2 open slots on the right side (top slot is 1/4" and the one underneath is 1".   There is also a 1" X 3/4" open slot for camera lens and one small 1/4" open button slot on the lower right as shown on the photos.  I do not know what phone this will fit.
