These notepads were sold by a local PTA to raise funds. The top reads A NOTE TO SCHOOL with lines below to fill in designated for the date, teacher name, parent signature, student name then a list of options to check with space for details including reasons for lateness, illness, pickup changes, trip request other. There is one small scribble on the back of the pad itself.
